 A FULLY COMPLETED REGISTRATION FORM MUST BE SUBMITTED FOR EACH INDIVIDUAL TEAM.

ORGANIZATIONS REGISTERING MULTIPLE TEAMS NEED TO MAIL (OR EMAIL) A FORM FOR EACH TEAM.

NO TEAM WILL BE ADMITTED TO THE TOURNMANENT WITHOUT A FORM.

Registration Packets should be picked up before your first game.

Packets will be in the gym where your first game is scheduled. 

Someone from your team needs to pick up your team's schedule and coupons for the goody bags.

CONTEST: For each team that brings at least 25 pairs of used eyeglasses for recycling

the team will be entered into a prize drawing for extra special goody bags for the girls
